What Is IVF?


IVF, known as In-Vitro Fertilization, is the reproductive technology, which assists a woman in conceiving. The IVF treatment is used to develop fertilized embryos outside the body. This process involves the extraction of the ovum from the ovaries, during women’s ovulation days and then fusing the ovum with the sperm at a microbiological laboratory. The fertilized embryo is then transmitted to the uterus of the woman.

Conditions and Techniques


Using Assisted Reproduction Techniques
In assisted reproduction techniques, fertility experts use artificial means to achieve pregnancy in a laboratory. The essential methods used are Intrauterine Insemination (IUI), In-Vitro Fertilization(IVF), IntraCytoplasmic Sperm Injection(ICSI). While performing ART techniques, both a woman's egg and a man's sperm are mixed to make embryos. After the formation of embryos, it is then implanted inside the woman's uterus. IVF is the most effective type of ART.
For complex cases, ART procedures can be used as donor eggs, donor sperm, or previously preserved frozen embryos. Assisted reproductive technology procedures involve the process of surgically removing eggs from a woman's ovaries, combining them with man sperm in the laboratory, and transferring the embryos into the woman's uterus or for surrogacy purposes. ART does not include treatments in which only sperm are handled or procedures in which a woman takes medicine only to increase fertility.

Using Natural Cycle IVF (In-Vitro Fertilization)
Natural cycle IVF (in vitro fertilization) is almost similar to IVF, but without the use of medications to stimulate the ovary to form multiple ova or eggs. This technique involves accumulating and fertilizing the single ova released during a woman's monthly cycle and preventing the use of any fertility drugs while performing the treatment. In recent years it has been found Natural cycle IVF has made a resurgence and gained popularity among infertile couples and worldwide.
Natural cycle IVF came out as an alternative to conventional in vitro fertilization for both normal and poor responders. It has several advantages over standard IVF(In-Vitro Fertilization), such as eliminating the risk of ovarian hyperstimulation syndrome (OHSS), more importantly preventing multiple pregnancies through restricting the development of multiple embryos.

Using Assisted Hatching Of Embryos
Assisted hatching of embryos is a procedure that is carried in conjunction with IVF treatment at FirstStep IVF. IVF involves mixing a woman's ovum or eggs with a man's sperm in a microbiology laboratory. Eggs are considered fertilized only when a sperm succeeds in penetrating the egg or ova and hence the formation of a zygote takes place.
But when we step towards the IVF procedure, the fertilized eggs are monitored in the laboratory for three to six days and develop into embryos. The best embryo is then placed into the woman’s uterus helping the woman to become pregnant or the embryo can be frozen in the egg bank for future use. The embryo naturally breaks out of the outer layer of the shell as it grows.
Sometimes, embryos are unable to escape healthily from the outer covering of eggs. In that case, the doctor may ask the laboratory to make a small crack in the outer shell of the embryo or artificially weaken it by making a small hole in the zona pellucida before placing it into the woman’s body.
In this technique, the IVF embryologist uses micromanipulation, involving a microscope, to create a tiny rupture on the outer layer of the eggshell. Through assisted hatching, embryos expand and are implanted into the uterine wall, which leads to a successful pregnancy.

Using ICSI (IntraCytoplasmic Sperm Injection) Technique
The technique of ICSI involves the treatment of male fertility. The intracytoplasmic sperm injection is an IVF procedure in which a single sperm cell is directly injected into the cytoplasm of an egg. ICSI technique is used to prepare the gametes for the containment of embryos that may be transferred to a maternal uterus. Need for ICSI technique is when a man's sperm can't fertilize a woman's egg, for successful fertilization the head of the sperm must be attached to the outside of the egg, when attached, the sperm pushes through the outer layer to the inside of the egg, hence increasing the chances of fertilization. Sometimes the man's sperm cannot penetrate the outer layer, for different reasons. The egg’s outer layer may be thick to penetrate or the sperm may be unable to swim inside the egg. Hence in these cases, we perform an intracytoplasmic sperm injection (ICSI) procedure which can be done along with IVF which helps to fertilize the egg. During the intracytoplasmic sperm injection technique, a single male's sperm is directly injected into the cytoplasm of the egg.
Once fertilization occurs, the fertilized egg or an embryo grows in a laboratory for 1 to 5 days before it is transferred to the woman’s uterus.FirstStep IVF clinic in Delhi has helped many couples through ICSI.

Using Recurrent IVF Failure
Despite giving endless successful fertility results for infertile couples, some laboratory-developed embryos don't end up with successful implantation. In these circumstances, we treat couples with the specialized technique of Recurrent IVF failure.
